What is Case Management?
Many people have heard of case management, but they might not know what it is or if it’s for them. Case management is a service offered by a healthcare professional to support individuals and families in meeting both their health and mental health needs. Case managers work in different settings such as hospitals, community service agencies, for health insurance companies, and yes, even private in practice.
You might be wondering what exactly a case manager does. The short answer is, that we work with people with complex health and/or mental health challenges to get connected with resources, provide support with managing needs, and support with managing activities of daily living. At the end of the day, it can be anything that helps you to manage your life better.
This can mean supporting with scheduling and attending doctors’ appointments, as well as keeping track of health needs. This can mean supporting you with keeping a schedule so that you can get through your day more smoothly and complete tasks that you are having a hard time completing, or managing your day for mental health reasons. Perhaps you are struggling with managing certain aspects of your everyday life like finances, we can refer you to a resource that can help you with that or help you build and stick to a budget. This can even mean supporting you with keeping up with your medications. The options are very personalized and really depend on what your needs are.
Next you might be asking, ‘well what kind of person might benefit from case management.’ Often times people with more complex health and mental health needs can benefit from case management. This could be someone with a traumatic brain injury who has a caregiver that needs support managing their loved one’s care. It could be someone that has autism and is having trouble managing day to day aspects of their lives. It can also be someone that is going through a very hard time in their life and needs help getting things back on track.
The point is, that lots of different people can benefit from case management and that case management can be used in lots of creative ways in order to meet your needs.
